Initial Screening Procedure
- Initial phone screen with referring person, parent, and/or youth, to determine eligibility for program and availability of an appropriate home for placement.
- Gather pertinent records of past treatment and/or talk to specific treatment providers
- Arrange a meeting, visit to perspective home, or phone discussion with identified Professional Treatment Facilitator, if desired
- Arrange a face to face assessment of referred youth, if deemed necessary prior to placement decision
- Discuss financial obligations
- Finalize placement decision and set intake time, date, and location
Matching
Matching and placement are primary to a successful admission. There will be maximum consideration given to you as the parent (s) of any youth being considered for admission and total control over the decision to place. You will be able to meet or talk with the Professional Treatment Facilitator (substitute parent) prior to any decision to place in that home. If feasible for you, a visit to the home and discussion with the PTF will be available prior to a decision to place.
Intake Procedure
- Meet at Phoenix Home’s office (PTF, youth, staff, and primary family when possible) Video conferencing available
- Phoenix Home’s staff will gather all relevant testing and diagnostic materials as well as full disclosure of social and clinical treatment issues
- Orientation to program, expectations, and rules
- Review and signing of all required documents
- Set up communication and visitation plan
- Finalize financial agreement
- Proceed to PTF home
