Leo entered Phoenix Homes care on September 2004 and was successfully discharged on July 2005. Leo entered our care as a Juvenile Justice youth due to a delinquent act of theft with inappropriate peers. He appeared to engage quickly with his foster family and has formed a lasting bond with them. the foster family introduced Leo to new experiences during his stay with them. He also achieved many different things that included receiving his GED through the County GED program, receiving and maintaining a full-time job after receiving his education, and passing the entrance exam to the United States National Guard. Leo entered adulthood during his Phoenix care as well.
The foster family invited Leo on all family outings and took him with them on their family vacation to Florida with Phoenix and DCS approval. Leo had never been to Florida or seen the ocean. He also enjoyed many other experiences with the family that included going to minor league baseball games, eating out at restaurants, and participating in family holiday events. Leo appeared to feel "included" in every aspect with his foster family. He appeared to be a role model for his foster siblings and recognized his poor decision making skills in the past.
Leo participated fully in his mental health individual counseling and outpatient alcohol and drug counseling with a successful discharge letter obtained by his alcohol and drug counselor. He demonstrated leadership with his actions and achievements and is presently in the United States National Guard because of his dedicated efforts.
Leo maintained successful contact with his biological mother through his Phoenix care and was successfully discharged to a relative placement before his military leave date for boot camp. He valued his biological family and enjoyed every approved visit with them.
